What failed was cloture, the motion to proceed to debate. It needs 60 votes. It got 49. Four Republican senators switched at the last minute, and one of them, Senator Tillis, did something deliberate: he wanted the bill to pass, saw the vote was going to fail, and changed his own vote to no. Only a senator who voted no can ask for a re-vote. He blocked it so he could keep that door open.
